Changes to the lips are among the first signs of facial ageing that people notice, and they are also among the least discussed. Lips that were once naturally full can appear thinner, flatter, or less defined over time. Asymmetry that was barely visible at twenty can become more noticeable at thirty-five or forty. These changes are not cosmetic vanity. They are the result of predictable biological processes that affect the tissue, bone, and muscle of the face as we get older.

Understanding what causes these changes, and knowing what questions to ask before considering any next step, puts you in a much better position when you attend a consultation.

What Causes Thin or Uneven Lips?

The lips are composed of several tissue types, including skin, muscle, subcutaneous fat, and mucosa. Each of these changes at a different rate with age, which is why lip appearance shifts gradually rather than suddenly.

Collagen and hyaluronic acid depletion are the primary drivers of thinning. Collagen provides structural support to the lip tissue, while hyaluronic acid retains moisture and contributes to volume. Both are produced in progressively smaller quantities from the mid-twenties onwards. As levels decline, the lips lose their plumpness and the skin of the upper lip, in particular, becomes thinner and less defined.

Bone resorption contributes to changes in lip appearance in ways that most people do not associate with the lips at all. The maxilla and mandible, the bones of the upper and lower jaw, gradually reduce in volume from around the age of forty. Because the soft tissue of the lips sits over this bony foundation, bone resorption causes the lips to appear to roll inward and flatten, even when the tissue itself has not significantly changed. This is why lip appearance can shift noticeably in the forties and fifties without any direct change to the lips themselves.

Muscle changes also play a role. The orbicularis oris, the ring of muscle that encircles the mouth, gradually weakens with age. Reduced muscle tone contributes to a loss of the crisp border between the lip tissue and the surrounding skin, making the vermillion border, the defined edge of the lip, appear less distinct.

These factors combine at different rates in different people, which is why the pattern and timing of lip changes varies so significantly from person to person.

Natural Volume Loss Over Time vs. Lip Shape You Were Born With

It is worth distinguishing between changes that occur over time and characteristics that have always been present but that you are noticing differently now.

Some people have naturally asymmetrical lips. A slight difference in the fullness of the upper versus lower lip, or a minor variation between the left and right sides, is extremely common and entirely within the range of normal anatomy. Asymmetry of this kind may become more noticeable with age, not because it has worsened significantly, but because the surrounding tissue has changed and the overall balance of the face has shifted.

Other people notice that their upper lip in particular appears to have become shorter and flatter with age. This is often a combination of the bone and collagen changes described above, along with a gradual thinning of the vermillion border that makes the lip appear to recede into the face rather than projecting forward.

Understanding which of these is driving your concern is part of what a good consultation should address. The causes of change over time and the causes of naturally asymmetrical anatomy are different, and they are worth understanding separately before considering any next steps.

Non-Clinical Ways People Address Lip Appearance

Before considering any professional consultation, many people explore at-home and non-clinical approaches. These vary in their effectiveness depending on what is driving the concern.

Lip care and hydration can improve the surface appearance of the lips meaningfully. The lip skin does not contain sebaceous glands and does not produce its own oil, which makes it more vulnerable to dryness and dehydration than the rest of the face. A consistent lip balm or treatment containing ingredients such as ceramides, niacinamide, or peptides can improve the texture and condition of the lip skin, which in turn makes the lips look more defined and comfortable.

SPF application to the lip area is consistently underused. The upper lip in particular is directly exposed to UV radiation and accumulates sun damage over time. This damage contributes to fine lines above the lip border and worsens the appearance of thinning. A lip balm with SPF 30 or higher, applied daily, is one of the simplest and most genuinely effective things you can do to slow visible changes in this area.

Makeup techniques offer cosmetic correction without any clinical element. Lip liner applied precisely at or just outside the natural border can create the impression of more definition and volume. Lighter, warmer tones tend to make the lips appear fuller than very dark shades. These approaches do not address the underlying biology, but they are effective tools for managing appearance on a day-to-day basis.

When It’s Worth Discussing With a Practitioner

For some people, at-home approaches are sufficient. For others, the degree of change or asymmetry has reached a point where they want to understand what professional options exist, and what the process of exploring them actually involves.

A practitioner consultation is worth considering if you have noticed a change that has affected how you feel about your appearance, if the change has been gradual enough that you are uncertain what is driving it, or if you want an accurate, clinical assessment of what is happening structurally before deciding on any next steps.

A consultation is not a commitment to any treatment. It is an opportunity to have your concerns assessed by someone with the clinical knowledge to explain what is causing them and what, if anything, can be done.

What a Lip Consultation at VicLaser Covers

A consultation at Victorian Laser and Skin Clinic is structured around understanding your anatomy and your concerns before anything else. You will not be presented with a treatment plan without first having a thorough assessment and an open conversation.

Your practitioner will assess the structure and symmetry of your lips, examine the surrounding tissue including the skin above the upper lip and the corners of the mouth, and ask about the changes you have noticed and when they began. They will also discuss your overall facial anatomy, because lip appearance cannot be properly assessed in isolation from the rest of the face.

You will have the opportunity to explain what specifically concerns you, whether that is overall volume, asymmetry, the definition of the border, the appearance of the upper lip when relaxed, or something else. The more specific you can be about what you have noticed and what you would like to understand better, the more productive the conversation will be.

The consultation will also cover your general health, any medications you take, and your history with any previous skin or cosmetic treatments. This information is relevant to any clinical recommendation and is gathered before any treatment discussion takes place.

Questions Worth Asking Before Any Cosmetic Treatment

Whether your consultation at Victorian Laser and Skin Clinic leads to a treatment recommendation or not, knowing what to ask puts you in control of the decision.

What is causing the change I have noticed?

A good practitioner can explain the anatomy behind what you are seeing. Understanding the cause helps you evaluate whether any proposed treatment is actually addressing the right thing.

What are all the options available to me?

Including non-clinical options and the option of doing nothing. A trustworthy practitioner will present the full picture rather than steering you toward a specific outcome.

What results are realistic, and over what timeframe?

Avoid any consultation that involves absolute promises about outcomes. Realistic expectations are grounded in your individual anatomy and are communicated honestly before any decision is made.

What happens if I choose not to proceed?

The progression of natural volume loss with age is entirely manageable, and many people choose not to pursue clinical treatment. Understanding what the natural trajectory looks like gives you an informed baseline for any future decision.

What qualifications does the person performing the treatment hold?

At Victorian Laser and Skin Clinic, all cosmetic treatments are performed by AHPRA-registered practitioners with relevant clinical qualifications. You are entitled to ask about this directly and should feel comfortable doing so.
